1. Sponsor

This giveaway is sponsored by Elite Orthodontics Studio, located at 350 Old Country Road, Suite 105, Garden City, NY 11530.

2. Giveaway Name

The name of this promotion is the Free Orthodontic Case Giveaway.

3. Entry Period
